Please give us your JOB TITLE (because only medical and social care professionals have access to this training) , the name of the place where you work and your address (street, CITY, COUNTRY, for the billing). Please note that this form is just a pre-registration, we check that you meet the prerequisites for the course, then we send you a quote and the detailed course programme. All you have to do is sign and return it to us to confirm your enrolment.
Then the paid invoice will validate your registration and give you access to the training.
